Community-led Pathways to Care
Support Elders and Older Aboriginal and Torres Strait Islander People to:
Understand the Aged Care System, including identifying the level of care and services required. Receive greater local support, linking services with local knowledge, access the right aged care services to meet their needs and to engage with all components of the aged care system. Reduce barriers across the aged care journey experienced by Aboriginal and Torres Strait Islander People.
Increase the number of Aboriginal and Torres Strait Islander people accessing aged care services.
Increase the number of Aboriginal and Torres Strait Islander people recieving care on country.
Increase the level of understanding of the needs and experiences of Aboriginal and Torres Strait Islander people in Aged Care.
Support we offer:
Connect with local Elders and Older Aboriginal and Torres Strait Islander people, and their families and
the community to raise awareness of the Aged Care support services available for our people.
Support navigation and access to appropriate aged care sevices for Elders and Older Aboriginal and
Torres Strait Islander people, this will include registration with My Aged Care as required.
This program is funded by NACCHO to provide Aged Care Cultural Support to ensure our
Aboriginal and Torres Strait Islander mob 50+ and their families are supported to understand,
navigate, and acce.
